Abstract

Lipedema is a chronic and progressive condition that predominantly affects women, characterized by the abnormal and symmetrical accumulation of subcutaneous adipose tissue, usually in the lower limbs, accompanied by pain, increased sensitivity, edema, and persistent inflammation. This condition directly interferes with the quality of life of patients, making diagnosis and appropriate clinical management difficult. Among the available therapeutic approaches, manual lymphatic drainage (MLD) stands out for helping to reduce discomfort and improve lymphatic circulation, promoting symptomatic relief and well-being. The association of this technique with topical formulations containing anti-inflammatory and toning active ingredients represents a promising strategy to enhance its effects, especially when combined with nanostructured approaches capable of optimizing the delivery of the active ingredients. This study aimed to develop and evaluate a cream formulation with anti-inflammatory and firming properties, intended for application during MLD in the dermato-functional care of patients with lipedema. This is a benchtop experimental study conducted at the Pernambuco Faculty of Health, with support from the University Pharmacy (Farma FPS), from July to September 2025. The formulation was developed using an oil-in-water (O/W) base with Polawax® (INCI: Cetearyl Alcohol (and) Polysorbate 60), incorporating the active ingredients Ginkgo biloba, hydroxyprolisilane (INCI: Methylsilanol Hydroxyproline Aspartate), and cafeisilane C (INCI: Caffeine (and) Siloxanetriol Alginate (and) Butylene Glycol), selected for their antioxidant, anti-inflammatory, and microcirculation-stimulating properties. Physicochemical, stability, and microbiological tests were conducted, ensuring the quality and safety of the product according to current regulatory standards. The results demonstrated that the formulation presented good stability, a homogeneous appearance, an adequate pH, and a pleasant texture, which facilitates its application during manual lymphatic drainage (MLD) procedures. Thus, the developed product proves to be a viable alternative for clinical use, contributing to the integration between pharmaceutical care and dermato-functional physiotherapy treatment in the management of lipedema.
